Use Form 1099-R Payer or State Copy 1 to print and mail payment information to the State (if required) or for payer record copies. 1099-R Forms are 2up on an 8 1/2" x 11" sheet with no side perforations and printed on 20# laser paper. Mail in batch to the state if needed. Order by number of forms, not sheets.
